By checking this box, I grant permission for my child to participate in the Communiversity Youth Program at the designated times and places. I also understand that my written permission will be required before my child may participate on any field trips provided by the Communiversity programs. I also understand that my written signature will authorize release of academic records that will lend assistance to the overall positive development of academic performance. A $50 application fee per academic school year for students and active parental participation is required.
An application fee of $50 paid must be paid in full before your child may be accepted into the Communiversity Youth Program. Make a money order or certified check payable to Communiversity. Fees can be sent to: Director of Communiversity, Sonja Haynes Stone Center for Black Culture and History, CB#5250, University of North Carolina at Chapel Hill, Chapel Hill, NC 27599-5250.
